Replace a lost or stolen passport
Report it first — a reported passport is invalid immediately and cannot be used by anyone else. Replacement is always in person.

Step 1
Report the loss immediately
Submit form DS-64 online or by phone as soon as you realize it's gone. Reporting invalidates the book so it can't be misused.
Step 2
Complete a DS-11 as well
A replacement is treated as a new application. Fill out the DS-11 but leave it unsigned.
Step 3
Re-prove citizenship and identity
Without your old passport you'll need a certified birth certificate or naturalization certificate, plus a photo ID and photocopies of both.
Step 4
Apply in person
Bring the DS-11, DS-64, documents, a new photo, and two payments to an acceptance facility. If you travel within 14 days, book a regional agency appointment instead.
Step 5
Expect extra scrutiny if it happens twice
A second loss within a short period can trigger a limited-validity passport or a request for a written explanation.
