Fill out Form DS-64 and mail it to:

In Person

Report in person when applying for a new passport at any passport acceptance facility.

Important Information
  • Do not report a passport lost or stolen if it has expired.
  • Once you report your passport lost or stolen, it is invalidated and cannot be used for travel.
  • If you find your passport after reporting it lost/stolen, you cannot use it for travel.
  • Reporting protects you from someone else using your passport.
